Which insurances are accepted?
We accept and bill all major insurance companies. We make every effort to ensure that your specific insurance product is accepted by our physicians. In the event that insurance does not cover the visit or services, the patient would bear the financial responsibility.
What should I bring to my first appointment?
Identification, insurance cards, form of acceptable payment (if necessary), and any prior medical records that you would like the physicians to review or have in our system. If you have obtained imaging (CT, MRI, XRays), please bring the CD/DVD for our physician’s review and records.
Do I need a referral?
Referral requirements depend on your individual insurance plan. You can usually find this information on your insurance card, or by calling the member services number listed on the back of the card for details about your specific coverage.
What is your financial policy?
All copayments, deductibles, coinsurance amounts are required prior to or immediately after service. More information can be found here.
What is the practice policy for cancellations?
We appreciate your courtesy in informing us of cancellations at least 24 hours prior to your appointment. “No Show” appointments may face a charge in order to allow for our patients to be seen in a quick, convenient, and timely fashion.
